Dear Doctor Said:

Upon recommendation of the Intern and Resident Staff Committee, it gives me great pleasure to confirm that you have been accepted as Intern in the Emergency Department from February 21st to June 14, 1972 provided you are eligible for registration on the Educational Register of the College of Physicians and Surgeons of Ontario.

You will receive a remuneration of $7,000.00 per annum including a living-out allowance and food allowance. The Hospital will also supply and launder uniforms and pay 75% of the hospitalization and medical insurance premium as well as the annual registration fee of $5.00 to the College of Physicians and Surgeons of Ontario. Two weeks vacation is allowed over a twelve month period and may be taken during the internship, upon written recommendation of the Head of Department and approval by the Medical Director.

Will you please complete, sign and return immediately the enclosed Acceptance of Appointment form.

As you must be registered on the Educational Register of the College of Physicians and Surgeons of Ontario during your training here, I strongly urge you to write to the College, 64 Prince Arthur Avenue, Toronto 180, Ontario and make formal application to be registered during the above—mentioned period.

You must also be a member of the Canadian Medical Protective Association; therefore, you should apply immediately in writing at 1867 Alta Vista Drive, Ottawa, Ontario.
